Community Connector Service
The Ageing Well Services Community Connector Service will be available throughout the holiday period, excluding a closure period from 24 December and recommencement on 5 January.
Please note staffing levels will be reduced throughout January, and there may be some delay in responding to messages and calls. We are here to help you so please let us know if you have an urgent matter so that we can prioritise returning your call.
Community Connectors
City of Ballarat has a dedicated team that can assist residents 55 and over and their carers to connect with the local community and navigate services such as My Aged Care.
Our service is FREE and can provide personalised information and drop-in support to empower you to age well in Ballarat.
