The application form can be picked up from Customer Service or sent via post on request.
The form needs to be completed and signed by your doctor, and returned to the Phoenix Building, either in person or in the pre-paid envelope. There is a $6.00 fee and payment accompanying the form will be processed immediately.
The permit will be issued to you and must be clearly displayed in the front windscreen of your car. Failure to properly display the permit may result in a fine.
Permits are valid for up to two years. Applicants with a permanent disability will be issued with a permit for three years. At the end of this period the permit holder may be required to complete a new application to renew the permit.
Temporary permits will be issued for a disability that is not likely to improve within six months and a further medical certificate must be presented for renewal.
