DISABLED PARKING PERMIT
Counci's Disabled Parking Permit forms can be downloaded here:
These forms can also be obtained from the Phoenix Building (25 Armstrong Street South, Ballarat), or can be posted out at your request.
The form needs to be completed and signed by your doctor, and returned to the Phoenix Building, either in person or in the pre-paid envelope. Payment accompanying the form will be processed immediately, and will save delays in having to chase up the fee.
There is a $5.00 fee and the permit is valid for up to two years. The permit will be issued to you and must be clearly displayed in the front windscreen of your car.
Failure to properly display the permit may result in a parking fine.
