Immunisation Records

Comprehensive immunisation records are kept by the City of Ballarat where clients have received immunisations through our service, however, our records do not include immunisations carried out by other health providers eg. your local doctor or Community Health Centre.

Australian Childhood Immunisation RegisterToddler being immunised

The Australian Childhood Immunisation Register (ACIR) is a national database that keeps a record of your childrens' immunisations up to the age of seven years. Immunisation providers (Council or your doctor) report your childs immunisations to the ACIR. A history statement of all of your childs immunisations is automatically posted to you at one, two and five years of age.

The Australian Childhood Immunisation Register now supplies a history statement that acts as a school entry immunisation certificate. The statement is automatically posted to you once your child has completed the four year old immunisations and has completed all required childhood immunisations. If for some reason your childs immunisation records are incomplete, missing, or your child has never been vaccinated, contact Council's Immunisation Coordinator (see contact details below) who will be able to assist you in obtaining a school entry immunisation certificate.
