Immunisation sessions are held on the first and third Wednesday of each month between 4.00pm and 5.30pm at the Eastwood Leisure Centre, Eastwood Street, Ballarat. No appointments are necessary. See also Maternal & Child Health Care - Ballarat Central.

New Babies Immunisation 2008
Immunisations will be available by appointment at The Phoenix, 25 Armstrong Street South, between 9.30am & 12.00pm. Appointments can be made by phoning 03 5320 5850.

Please note: The Australian Childhood Immunisation Register (ACIR) is a national database that keeps a record of your childrens' immunisations up to the age of 7 years. Immunisation providers (GP or council) report your child's immunisations to the ACIR. A history statement of all your child's immunisations is automatically posted to you at 1, 2 and 5 years of age.
The Australian Childhood Immunisation Register now supplies a history statement that acts as a school entry immunisation certificate. It is automatically posted out to you once your child has completed the 4 year old immunisations and has completed all required childhood immunisations. If for some reason your child's immunisation records are incomplete, missing or your child has never been vaccinated, contact your local council immunisation service who will be able to assist you in obtaining a school entry immunisation certificate.
